The processing of personal data is governed by the General Data Protection Regulation 2016/679 (GDPR) which gives more rights to you as an individual and more obligations to organisations holding your personal data.

The type of personal information I collect

I currently collect and process the following information:

How I get the personal information and why I have it

Most of the personal information I process is provided directly by you for one of the following reasons:

1) For contact purposes. By submitting an enquiry form through my website you provide your telephone number, email address, your name and sometimes some relevant medical information relating to your enquiry. I use this information for contact purposes to arrange client appointments and to gain an initial understanding as to your individual reason for treatment.

 

2) For appropriate assessment and treatment purposes. On your first appointment I will carry out an initial consultation, detailing your current symptoms and reason for treatment. I will ask you for information about your medical history including past and present medical conditions, any medications you take and some general lifestyle questions. Following treatment, I will record on your treatment record any relevant findings from your assessment or treatment, what treatment you had and any advice I gave following treatment

 

Your data protection rights

Under data protection law, you have rights including:

Your right of access - You have the right to ask us for copies of your personal information.

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
